Consent to the processing of personal data

By ticking the box in the booking form, a parent or legal guardian of the child freely gives the Smilia clinic (ul. Piotrkowska 100, lok. 2, 90-004 Łódź) consent to process personal data on the following terms.

What data

Name, phone number, the child's age, the reason for contact. The reason for contact may include information about the child's health, and the consent covers it too (Article 9(2)(a) GDPR).

For what purpose

To get in touch, book the child's visit, confirm the details of the visit and answer a question.

What is done with the data

Collection, recording, storage, use for contact, passing on to the extent needed to run the website and deliver messages, deletion.

For how long

12 months from the date the request is sent, or until consent is withdrawn if that happens sooner.

How to withdraw

A message to info@smilia.jimagie.com asking for the data to be deleted. Withdrawal does not affect the lawfulness of processing before it. After withdrawal the data is deleted unless there is another lawful reason to keep it.

How consent is recorded

When the form is sent, the date and time, a consent mark, the version of this text (of 03.10.2026) and the page it was sent from are saved.
