Privacy policy
This page describes how the clinic handles the personal data left in the booking form on this website.
Who processes the data

What data is collected
The booking form collects:
- the name of the person sending the request;
- a phone number;
- the child's age;
- the reason for contact chosen from a list;
- the page of the website the request was sent from.
A record of consent is saved with the request: date and time, a consent mark and the version of the consent text. The IP address is not stored.
Why the data is needed
The data is used to get in touch and book a child's visit, to confirm the details and to answer a question. The data is given voluntarily, the form cannot be used without it, but a visit can always be booked by phone. The legal basis is explicit consent (Article 6(1)(a) GDPR). The reason for contact may include information about the child's health, so consent is also given under Article 9(2)(a) GDPR. There is no automated decision-making or profiling. The data is not used for advertising mailings.
Who can see the data
Only authorised clinic staff who need the data for booking can see it. A hosting provider and a mail service are used to run the website and deliver messages, and they receive data only to the extent needed for that. The data is not sold or passed to third parties for their own purposes.
How long the data is kept
The request and the record of consent are kept on the website for 12 months from the date of sending and then deleted automatically. The copy of the request that reaches the clinic's mailbox is kept there for no longer than 12 months. At a person's request the data is deleted sooner.
Rights
Everyone has the right to:
- receive information about their data and a copy of it;
- have inaccurate data corrected;
- ask for the data to be deleted or its processing restricted;
- receive the data in a commonly used format and pass it to another controller;
- withdraw consent at any time by writing to info@smilia.jimagie.com. Withdrawal does not affect the lawfulness of processing before it;
- lodge a complaint with a supervisory authority: the President of the Personal Data Protection Office (Poland).
Browser data and third-party services
The website stores in the browser only the choice of language (for a year) and the choice made in the consent notice for third-party services. Third-party services, such as a map, are switched on only after consent. Without consent the website does not contact outside servers. The choice can be changed at any time with the "Consent settings" link at the bottom of the page.
Consent and changes
